Prevention
Preventive care. For patients who want a dental home and fewer unexpected problems. 15% off eligible treatment. Or $390 a year paid upfront, 2 months free.
- 2 cleanings a year plus emergency exams
- 2 exams and 2 cleanings per year
- Annual bitewings and necessary periapical X-rays; panoramic or full-mouth X-rays every 3 years
- 2 fluoride treatments and an annual oral cancer screening
- Unlimited emergency exams with diagnostic X-rays, no visit fee

- Child under 14: $19/month or $190/year
Restorative
Restorative care. For active decay, broken teeth, or old dental work that needs attention. 25% off eligible treatment. Or $890 a year paid upfront, 2 months free.
- 3 cleaning or maintenance visits plus restorative care
- Everything in Prevention, plus:
- 3 cleanings or periodontal maintenance visits per year, as indicated
- 1 emergency palliative treatment per year for pain relief
- 1 composite filling per year, 1 to 2 surfaces
- No urgent-triage visit copay; one nightguard per year at 50% off
- 25% off eligible treatment
- Child under 14: $35/month or $350/year
Comprehensive
Comprehensive care. For a full treatment plan ahead: extractions, root canals, implants, or grafting. 30% off eligible treatment. Or $1,090 a year paid upfront, 2 months free.

- 2 composite fillings and 1 simple extraction per year
- Unlimited emergency palliative care and treatment planning visits
- Annual CBCT imaging when indicated; post-operative visits and suture removal
- Unlimited teledentistry consults; priority same-day scheduling and after-hours access, subject to availability
- 30% off eligible treatment
- Family add-on: $59/month or $590/year per additional member
Maintenance
Ongoing care. For patients who have completed active treatment and want to protect their dental health. A standalone plan, not an upgrade from Comprehensive. 20% off eligible treatment. Or $590 a year paid upfront, 2 months free.
- 4 maintenance visits plus protection for completed work
- 4 cleanings or periodontal maintenance visits per year, with exams and necessary X-rays as clinically indicated
- Annual implant evaluation and maintenance; localized antimicrobial therapy at 2 sites per year when indicated
- Denture adjustments and 1 soft reline per year; an occlusal guard every 2 years when indicated
- Recementation of existing crowns and bridges when clinically appropriate; annual full-mouth reassessment
- New fillings, extractions, root canals, implants, and replacement restorations are not included

- Family add-on: $39/month or $390/year per additional eligible household member
